Reason for the leave:

Dates of Leave:

The leave application should indicate the specific dates the employee will be away from work, including the start and end date of the leave.

Contact Information: 

The leave application should include the employee's contact information, such as their phone number and email address,

Backup plan: 

The leave application should include a plan for how the employee's work will be handled in their absence, such as who will be covering their responsibilities during the leave.

Supporting Documents:

 If the leave is for medical reasons the employee should attach the medical certificate or any other document that supports their leave request.


The leave application should be signed by the employee, either digitally or manually.
